Description of Anthracnose (Colletotrichum graminicolum):
Affects bentgrass and annual bluegrass. Anthracnose appears to have a yellow or bronze cast, from a distance. It may wilt rapidly during midday heat. The individual leaf blades fade from dark green... to light green... to yellow and then die, without distinct leaf spots.

Instructions:

For broad spectrum control, tank mix with a registered contact fungicide at the label rate. If disease is present, mix 2 fl. oz. of Quali-Pro Propiconazole 14.3 per 1000 sq. ft. with the label rate of the above mentioned contact fungicides.
